Location
From Toledo
We are located in the South end of Toledo at the intersection of S. Byrne Rd. and Glendale on the S.E. corner behind the Foundation Park Plaza. Our driveway is off of Glendale, 200 ft. East of S. Byrne Rd.
From Cleveland And East
Ohio turnpike (I-80/90) West to Exit 59. Go North on US Route 20 (Reynolds Rd.), stay to the right lane. At the first traffic light turn right (East) onto Heatherdowns. Take Heatherdowns approximately 2 ½ miles to Byrne Rd. When you pass two golf courses that are across from each other stay in the left lane as you approach Byrne Rd. Turn left (North) onto Byrne Rd., stay in the right lane and turn right at the first traffic light. Turn right into the first driveway on your right, drive straight back to Foundation Park Care Center.
From Chicago And West
Interstate I-80/90 West to Ohio Turnpike West to Exit 59. Follow the same directions as “From Cleveland and the East”.
From Detroit And North
Interstate I-75 South to exit 201-A (Maumee/State Route 25 South). Rt. 25 is also called Anthony Wayne Trail. Follow St. route 25 south past the Toledo Zoo, move to right lane. Go through the first traffic light past the Zoo and exit to the right onto Glendale Avenue, approximately ¾ of a mile past the Zoo. Take Glendale Avenue West through 5 traffic lights, approximately 2 miles. Go through the 5th traffic light and turn left into the next driveway. Go straight back to Foundation Park Alzheimer’s Care Center.
From Dayton / Cincinnati
Interstate I-75 North to US Route 23 North/I-475. US 23 North/I-475 to the Salisbury Road exit, turn right (East) off the exit. Take Salisbury Road approximately 2 miles to US Route 20 North (Reynolds Road). Follow the directions “From Cleveland and East.”
From Dayton / Cincinnati
Take US 23 North to I-75 North. Follow the directions “From Dayton/Cincinnati”.
